Testing for COVID-19 from January 31, 2022

Dear Colleagues,

Starting January 31, the mandatory testing of FAMU employees will be covered by self-testsThe self-tests will be available at the FAMU reception, where you will pick them up upon entry and carry them out at your workplace. Always register the result of the self-test immediately in this formFORM FOR REPORTING TEST RESULTS FROM OTHER HEALTHCARE SERVICE PROVIDERS AND FOR REPORTING SELF-TESTS TAKEN AT AMU, or ask the secretaries of your departments to enter your data into the form. Completion is essential to keep track of the fact that you have taken the test and can be physically present at the workplace. The self-test is valid for three days (including the day of the test). Those who have undergone laboratory-confirmed covid-19 disease and have not passed more than 30 days since a positive RT-PCR test for the presence of SARS-CoV-2, on the basis of which isolation was ordered, need not be tested.

In case of a positive result:

1. Please leave the workplace immediately.
2. Send an email to the dean's office at kristyna.sperkova@famu.cz, which will provide information for electronic reporting to the hygiene station. The following information is required for reporting: name, surname, nationality, birth number (date of birth for foreigners), number of the insured, health insurance company, place of residence in the Czech Republic, employee's phone, date of self-test + information on whether you want to issue an e-incapacity.

At the same time, we would like to ask you for a home office mode, as much as the nature of your work allows. Please make an arrangement with your supervisor about the presence at the workplace until the beginning of the summer semester. We also ask that any meetings take place online, not in person. The most important thing is that we can start the summer semester in as much as possible full-time, especially practical teaching.
